    Clinical Associate/ Care Coordinator

    Job description

    WHAT WE NEED YOU TO DO

    Patient management:

    • Perform health assessments on patients in order to identify individual patient risk factors
    • Perform interventions as pre-defined to influence individual patient risk
    • Actively participate in the management of complex patients by building good relationships with the patient and their family
    • Monitor patient progress through ongoing patient and clinician interactions
    • Promote behaviour change to ensure greater treatment compliance and condition management
    • Coordinate the integration of care including social services
    • Assist in navigating additional benefits
    • Arbitrate and resolve conflict and interface with problems that arise in patient care.

    Coordination of care:

    • Regular liaising and communication with the care team (including clinicians, hospital, family members, counsellors and admin staff)
    • Assistance in the documentation of clinical information
    • Maintain relationships between external treating providers, the consortium’s multidisciplinary team and the management team
    • Build teams in the healthcare industry – being able to inspire teamwork at all levels to ensure optimal outcomes for the patient and the team.
    • Understand the team’s operational processes in order to improvement.

    Clinical process development:

    • Understanding clinical process and engage with clinical data in an effort to understand the issues and possible solutions health system related issues
    • Provide clinical input into broad range of clinical developmental work including development of templates and processes to improve the working of the team and the patient outcomes they produce
    • Actively contribute to improvements to ensure success of Integrated Clinical Consortia.

    Qualifications and experience required:

    Appropriate Clinical Associate or Registered Nurse qualification;

    5 years industry experience with a minimum of 2 or more in the following:

    • 3 or more years clinical experience
    • Multidisciplinary team experience;
    • Care Management or similar experience
