    Care Coordinator

    Job description

    Key Purpose

    The Coordinator would play a key role in an integrated funder-provider programme designed to co-ordinate the care of a carefully selected group of DH members by actively identifying patients that meet our clinical entry criteria. The Coordinator would facilitate the care of these patients in an appropriate facility in collaboration with the referring doctor, patient, family and Scheme. This requires the building and maintaining of excellent relationship with facility staff and patients.

    Areas of responsibility may include but not limited to

    • Perform health assessments on patients in order to identify individual patient risk factors
    • Perform interventions as pre-defined to influence individual patient risk
    • Monitor patient progress through ongoing patient interaction
    • Promote behaviour change to ensure greater treatment compliance and condition management
    • Coordinate the integration of care including social services
    • Authorising of admissions/ treatment as per agreed protocols
    • Regular liaising and communication with the care team (Including physicians, hospital, therapists, family members, counsellors and admin staff)
    • Assistance in the documentation of clinical information
    • Maintain relationships between the acute hospital, the specialists and the rehabilitation facility and Discovery Health

    Education and Experience

    requirements

    essential:

    • Qualified Registered Nurse
    • Valid SANC Registration

    5 years industry experience with a minimum of 2 or more of the following:

    • Clinical Nursing Experience
    • Case Management or similar experience
    • Multidisciplinary team experience
    • Valid driver’s license and own transport
    • Microsoft Office

    requirements

    Advantageous:

    • Exposure to sub-acute /rehabilitation
    • 1 year Pre-Auth consultant, Pre-Auth Specialist or Clinical CRM Experience
