    CCMDD Coordinator - Mthatha

    Purpose of the position:

    • Will be responsible for assisting THC, district offices, relevant district management and health facilities with any Central Chronic Medicine Dispensing and Distribution (CCMDD) and Synchronised National Communication in Health (SyNCH) related queries. This includes Information, Communication and Technology (ICT); training of DOH District officials on SyNCH; and clinical queries such as guidelines and formulary management and queries on medicine regimens on SyNCH. The CCMDD Coordinator will also provide guidance and training to sub-district and district ICT staff on any health sector related matters - and will monitor the use of the CCMDD software system across health facilities.

    Minimum Requirements    

    • Qualified Pharmacist or Professional Nurse with South African Pharmacy Council (SAPC) or South African Nursing Council (SANC)
    • 5 years or more relevant experience after registration as a Pharmacist or Professional Nurse
    • 2 years in a managerial/supervisory role
    • Valid code 08 driver’s licence
    • Proficiency in Microsoft Office package

    Duties and Responsibilities    

    • Rendering quality pharmacy support, especially to the District Differentiated Care
    • Development and implementation of a district pharmacy support operational plan in co-operation with all stakeholders and grant partners.

    • Supervision of Pharmacist’s Assistants to ensure continuous availability of medicine to patients by using proper stock management and ordering systems.

    Facility Team Lead - Ngqeleni

    Purpose of the position:

    • The Facility Team Lead will be responsible for overall facility performance and an anchor to ensure all program deliverables are successfully achieved through allocation of LOE to support QI, Case Management, Viral monitoring, CLI, eLabs and RTCQI etc.  Supports DoH in terms of CLI aspects at SPI-RT Level 3 and 4 facilities according to a routine schedule (Level 3 light touch support - each facility will have engagement minimum once a month; Level 4 surveillance support - each facility will have engagement minimum once a quarter; SEAD will provide technical assistance to QI mentors in this capacity; Level 2 and below facilities are directly supported by SEAD, the technical assistance partner for CLI).
    • The project aims to institutionalise and strengthen local capacity through provision of technical, mentorship and direct service delivery support to TB and HIV services when required and interacts with Ward Based PHC Outreach Teams (WBPHCOT) and Community Based Organisations (CBO) as required.         

    Minimum Requirements    

    • Bachelor’s Degree or Diploma in Nursing Sciences in health sciences
    • A diploma in Midwifery
    • A qualification in Primary Health care (PHC) or clinical assessment will be an added advantage
    • A diploma in HIV Clinical Management and /or NIMART
    • Certified training in Adult Primary Care (APC)
    • QI trained and fully understands the methodology and its application
    • 5 years relevant experience in the health sector managing/implementing HAST programs with knowledge of HIV and TB treatment cascade

    Duties and Responsibilities    

    • Supervision of facility-based staff to ensure all program deliverables are fully achieved
    • Conducts skills audit exercise to identify training gaps.
    • Market CCMDD external pick-up points in communities
    • Focuses intensely on QI, Case Management, general CLI, viral monitoring through eLabs and suppressions, RTCQI,
    • Works collaboratively with Outreach Team Lead to ensure bidirectional referral, linkage and retention in care
    • Tracks facility and outreach performance against set TB and HIV clinical cascade indicators and advise accordingly on improvement strategies
    • Leads the development of QIPs working collaboratively with Facility Manager and Team to introduce change ideas followed by close monitoring of such intervention
    • Ensure staff understands the contents of standard operation procedures and implements accordingly
    • Ensures synergy between facility and outreach teams through sharing of daily activities
    • Conduct mapping exercise to understand the disease burden and deploy outreach teams including Community Health Workers (CHWs) accordingly
    • Conducts Siyenza site facilities with representatives of the Centers for Disease Control and Prevention (CDC) and visits to Phuthuma sites with officials from the Department of Health
    • Support and monitor the implementation of patient-tracking and referral systems
    • Supports mentoring facility and outreach team in data analytics, use and decision making as informed by their own performance
    • Accommodates and cooperate with Civil Society representatives visiting facilities in line with facility and community monitoring functions
    • Works with Facility Manager to effectively establish and coordinate functionality of facility and clinic committees

    Professional Nurse Coordinator - Mahikeng

    Purpose of Position

    • To provide a comprehensive Wellness mobile health service by providing HIV testing and counselling services linked to screening and diagnosing clients for TB/STI’s.     Enrolled Nurse (Paeds) - OR Tambo

    Purpose of the position:

    • Provides clinical and administrative duties including but not limited to; supervision of case officers, linkage of clients into care, supports case management solutions for adults and pediatrics in attainment of 2nd 95 and 3rd 95. Liaise with various stakeholders, district, health facilities and other government departments.

    Minimum Requirements    

    • Grade 12 plus Enrolled Nurse qualification
    • HIV and TB management trainings, WBPHCOT will be an added advantage
    • A minimum of 2 years’ experience in HIV and TB programs
    • Registration with the South African Nursing Council as an enrolled nurse

    Duties and Responsibilities    

    • Blood monitoring and collection as per required monitoring intervals
    • Be familiar with the ROAD TO HEALTH BOOKLET and ensure completion of all relevant section at each visit working closely with P/N
    • Align EPI visits with HIV management of children and adolescents including growth monitoring and development
    • Key entry point screening and testing of children and adolescents 0-19 years for HIV/TB/Covid-19 as per HIV Matrix of Intervention and HIV/TB/PMTCT guidelines, including Antenatal/Post-natal/Breastfeeding clients using IMCI Checklist/PATEST tool/HIV adult screening tool, handholding those tested positive and link them to care
    • Enrol all children and adolescents newly diagnosed with HIV on case management model, for continuity of care and support and facilitate decanting to Differentiated Models of care as per SOP, working closely with community teams and OVC partners
    • Provide preventative counselling for women tested HIV negative during ANC/breastfeeding/PNC and offer Pre-Exposure Prophylaxis (PrEP).
    • Offer index testing services for ANC/breastfeeding/PNC partners and provide HIVSS test kits where necessary
    • Monitor child development and immunisation status of all children <5 years irrespective of HIV exposure history
    • Periodically, conduct MCH clinical file audit, to ensure effective implementation of PMTCT guidelines and effective clinical management of clients.
    • Ensures that all children and adolescents 0-19 years are in optimum user-friendly formularies and regimen as per age & weight, to enhance adherence and VL suppression
    • Take VL blood as scheduled in PMTCT guidelines and enrolled all children and adolescents with VL >50 c/ml on enhanced adherence counselling (EAC), report any issues needing attention to P/N or VL champion
    • Weekly actioning of RFA list as appear on RFA dashboard/ elabs device.
    • When due refer all clients require psychosocial support, disclosure and adherence counselling to social worker via FTL/PN.
    • Facilitate decanting of eligible < 19 years clients and periodically evaluate clients due for script renewal, with assistance of FTL/PN
    • Support establishment of youth-zones and its activities to reach <19 years whom consulted on that youth-friendly corner
    • Work with social workers to establish youth clubs, Post Natal clubs, Viremia clubs and Family care days.
    • Participate in Operation Phuthuma Nerve Centre activities that contributes to MCH indicators, including MCH score card, where applicable
