Nurture Health is seeking a compassionate and dedicated Care Worker to join our rehabilitation hospital team. Reporting to the Nursing Manager, you will provide high-quality, dignity-centred care that supports patients throughout their rehabilitation journey. Your role will contribute to patient comfort, independence, and wellbeing while working collaboratively within a multidisciplinary healthcare team.
Key Responsibilities
Deliver safe, compassionate care by supporting activities of daily living, promoting patient dignity, and monitoring patients' clinical status in accordance with Nurture's nursing policies and systems.
Observe, document, and promptly report changes in patients' conditions while collaborating with nurses and the wider interdisciplinary team to ensure excellent patient outcomes.
Represent the nursing team professionally, contribute to a respectful patient experience, and actively participate in ongoing learning and professional development.
Requirements
Successfully completed a recognised Care Worker or Nursing Assistant qualification (or equivalent) with proven experience providing care in a hospital or frail care environment.
Strong observational skills, empathy, teamwork, and the ability to plan and adapt daily activities while recognising and reporting changes in patients' clinical conditions.
Commitment to the Caregivers and Health Promotion Council (CHPC) Code of Conduct, patient-centred care, and continuous professional development within a rehabilitation environment.
