Build strong relationships with referrers, hospitals, funders, patients, and families.
Collaborate with multidisciplinary teams to support patient-centred care.
Maintain accurate records and ensure effective communication throughout the admissions journey.
Represent Nurture Health professionally within healthcare networks and stakeholder engagements.
Requirements
Registered with SANC as a Professional Nurse, or registered with the relevant HPCSA board as an Independent Practitioner (Physiotherapy, Occupational Therapy, Speech Therapy, or Dietetics).
Experience in patient referral, assessment, and admissions within a multidisciplinary healthcare environment.
Strong clinical reasoning, communication, relationship-building, and organisational skills.
A passion for rehabilitation and advocating for optimal patient outcomes.
