Nurture Health is looking for a compassionate and motivated Rehabilitation Supporter to join our rehabilitation hospital team. Reporting to the Therapy Manager, you will support patients throughout their rehabilitation journey by assisting the therapy team, promoting patient independence, and contributing to a safe, respectful, and therapeutic environment. Your role is essential in helping patients achieve meaningful rehabilitation outcomes.
Key Responsibilities
Support therapy programmes and patient care activities in accordance with Nurture's therapy policies, procedures, and systems while promoting patient comfort, dignity, and independence.
Observe and promptly report changes in patients' clinical status, collaborate with therapists and the interdisciplinary team, and communicate relevant information to support effective rehabilitation.
Represent the therapy team professionally, report on daily activities and outcomes, and actively participate in learning and professional development.
Requirements
Successfully completed a recognised Care Worker or Nursing Assistant qualification (or equivalent) with proven experience providing care in a rehabilitation hospital or integrated care environment.
Strong observation, communication, and teamwork skills, with the ability to plan and adapt daily activities while recognising changes in patients' clinical condition.
Commitment to the Caregivers and Health Promotion Council (CHPC) Code of Conduct, patient-centred rehabilitation, and continuous professional development, with Level 1 knowledge of the World Health Organisation's Rehabilitation Competency Framework.
