    Community Programme Coordinator

    Purpose of the position:

    • Community Programme coordnator role will focus on establishing and maintaining partnerships with the CSS Medium and Small Grant CBOs and FBOs, local coordinating forum on health and or AIDS; and representing the AYP programme at those fora. This role is also ideally placed to manage and update the sub-district-based services directory for easy service plan referrals and report on the Human Rights Violations

    Minimum Requirements    

    • Diploma / NQF level 6 is essential
    • Minimum 2 years’ experience in lobbying/support/human rights or programming in the public health or HIV field
    • Experience in establishing partnerships within a CSS Medium and Small grant CBOs and FBOs

    Duties and Responsibilities    

    • Establishing, maintaining and supporting the rights and needs of AYP in selected district
    • Playing a key role in networking and engagement with district stakeholders
    • Conduct sensitisation training to stakeholders per quarter including reporting on human rights activities.
    • Co-facilitate community dialogues related to human rights and human rights violations of AYP to publicize, encourage reporting and minimize human rights violations
    • Participate in sub-district level meetings

    Health & Wellness Counsellor

    Purpose of the position:

    • The Health and Wellness Counsellor will be responsible for the Risk Assessment (CORE), HIV testing and referral as well as health education and condom distribution in the My Journey Programme.

    Minimum Requirements    

    • Grade 12) or equivalent to NQF 4
    • Minimum of 1-2 years HIV counselling and testing experience

    Service Delivery Support Member_Lusikisiki

    Purpose of the position:

    • To ensure compliance with all the domains of the National Core Standards and other relevant related legislative frameworks and policies. Will be responsible for the coordination and management of all QI projects within the Department of Health Programmes supported by THC in Chris Hani District.     Peer Educator

    Purpose of the position:

    •  To conduct out-reach activities to reach MSM community by providing accurate health information, mobilization of MSM to the mobile unit and linkage to care. The peer educator  is responsible for providing peer support to all patients already on HIV treatment, and those beginning their treatment journey. Providing telephonic support and follow-ups. The incumbent should also be tracing Pre ART, and treatment interrupters with the aim to bring them back and re-engage them to care. They will also be responsible for providing education and information around treatment and care to all patients. Providing information and support to participants (new and existing) in the decanting programme as well as rendering psychosocial support counselling and mentoring on a regular basis resulting in adherence and retention to care.

    Facility Team Lead - OR (Qumbu)

    Purpose of the position:

    • The Facility Team Lead will be responsible for overall facility performance and an anchor to ensure all program deliverables are successfully achieved through allocation of LOE to support QI, Case Management, Viral monitoring, CLI, eLabs and RTCQI etc.  Supports DoH in terms of CLI aspects at SPI-RT Level 3 and 4 facilities according to a routine schedule (Level 3 light touch support - each facility will have engagement minimum once a month; Level 4 surveillance support - each facility will have engagement minimum once a quarter; SEAD will provide technical assistance to QI mentors in this capacity; Level 2 and below facilities are directly supported by SEAD, the technical assistance partner for CLI).
    • The project aims to institutionalise and strengthen local capacity through provision of technical, mentorship and direct service delivery support to TB and HIV services when required and interacts with Ward Based PHC Outreach Teams (WBPHCOT) and Community Based Organisations (CBO) as required.         

    Minimum Requirements    

    • Bachelor’s Degree or Diploma in Nursing Sciences in health sciences
    • A diploma in Midwifery
    • A qualification in Primary Health care (PHC) or clinical assessment will be an added advantage
    • A diploma in HIV Clinical Management and /or NIMART
    • Certified training in Adult Primary Care (APC)

    Duties and Responsibilities

    Case Officer_Mqanduli

    Purpose of the position:

    • Responsible for implementing and monitoring the Case Management Model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care.

    Minimum Requirements    

    • Grade 12 certificate
    • Enrolled nurse qualification will be an added advantage 
    • At least 2 years’ relevant experience in counselling and testing
    • Adherence counselling
    • Basic HIV testing and counselling
    • Index case trailing
    • Able to communicate in English and isiXhosa is an advantage
    • Criminal Clear

    Duties and Responsibilities    

    • Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments
    • Capture clients with high viral loads, pediatric clients, pre-ART clients and pregnant women
    • Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care
    • Issue routine reminders according to the schedule of appointments by sending SMS reminders
    • Responding to messages from clients and crafting an appropriate and professional response
    • Refer patients for enrolment in support groups according to the patient’s preference
