Job Description

• The purpose of this position is to provide local coordination and support for bidirectional referrals capacities/systems in the Ehlanzeni and Nkangala Districts of Mpumalanga Province, under the USAID-funded and m2m-led Children and Adolescents Are My Priority project – as part of our expanding multi-million dollar five-year, a USAID -funded grant for orphaned and vulnerable children (OVC) which is in its fifth implementing year. This holistic programming is helping to build the capacity of the most vulnerable children and their entire families – whom are most affected by HIV to meet their basic needs, by removing economic barriers to HIV testing and treatment, reducing HIV stigma in the community, and improving education and knowledge in the community about HIV. 

Job Requirements
Key Performance Areas:

• Coordination / ensuring active & rapid linkages to HIV related services and case management interventions for children living with HIV (CLHIV) 
•    Actively support the new identification, tracing, and referral pathways for CLHIV – in tandem with USAID’s District Support Partners and DoH staff at high-volume health facilities across Nkangala or Gert Sibande districts – and ensure enrollment of CLHIV into m2m CHAMP case management programme.
•    Other activities that support outreach and strengthen linkages to/from health facilities in district
•    Other activities that support referrals /service delivery coordination for OVC and CLHIV

    Partner/stakeholder management 

•    Engage District Support Partners, DoH /health facility staff, local leaders, CBOs, NGOs, and key overnment partners (DoH, DoSD, DBE) and provide sensitization about CHAMP’s case management services and other packages of care

     Qualifications and experience:

•    Experience with supporting linkages and bidirectional referrals among health facilities and community-based programmes 
•    Experience with OVC preferred
•    Knowledge of community -based and/or case management delivery models 
•    Fluent in written and spoken English and competent at written communication and reporting writing
•    Proficient in basic computer skills 
•    Knowledge of DOH systems and protocols
•    Drivers license
