    HIV Ambassador_Mthatha

    The core responsibility of the HIV Ambassador is community mobilisation and demand creation towards the achievement of the UNAIDS 95-95-95 targets. The approach entails interfacing with proximal facilities to get a sense of their priority programme activities, then embarking on awareness campaigns and social activations to promote and advocate for those priority programmes. Furthermore, the HIV Ambassador would interact with key influencers in the communities (youth formations, traditional leaders, ward counselors) to create demand for health services. Additionally, the HIV Ambassador would assist the facilities in the tracing of patients on missed appointments lists in communities around their places of stay. The target population includes: ART & TB clients who have missed their appointments, mother-baby pair, unsuppressed viral loads and elicit index client information for follow up and pregnant women.

    Minimum Requirements    

    • Minimum of grade 10 certificate 
    • Living openly with HIV 
    • A community advocate on health issues especially HIV/AIDS 
    • Able to communicate in English and isiXhosa is an advantage 
    • At least two (2) years’ relevant experience in counselling and testing 
    • Basic HIV counselling and testing

    Duties and Responsibilities    

    • Conduct health education in communities on HIV, AIDS & TB as part of the 95-95-95 strategy 
    • Source missed appointment list from Data Capturer for daily tracing 
    • Create health awareness campaigns in identified communities on the importance of 95-95-95 status for HIV and TB, stigma and discrimination and on treatment adherence
    • Leverage social platforms (imbizo, youth gatherings, LAC, DAC etc.) to promote 
    • Refer patients for enrolment in support groups according to the patient’s preference 
    • Work as part of a multi-disciplinary team to optimize targets 
    • Maintain client confidentiality 
    • Provides administrative support in terms of daily reports and report writing.

    Enrolled Nurse_OR Tambo

    Purpose of the position:

    • Provides clinical and administrative duties including but not limited to; supervision of case officers, linkage of clients into care, supports case management solutions for adults and pediatrics in attainment of 2nd 95 and 3rd 95. Liaise with various stakeholders, district, health facilities and other government departments.

    Minimum Requirements    

    • Grade 12 certificate
    • Must have an Enrolled Nurse qualification
    • Must have 2 years’ experience in TB & HIV Programs
    • HIV and TB management trainings, WBPHCOT will be an added advantage
    • Must be SANC registered
    • Must have basic computer skills – Microsoft office
    • Must be able to communicate in English, isiXhosa languages
    • Must have a Clear Criminal record

    Duties and Responsibilities    

    • Provision of mentorship for HIV and TB screening and linkage to HIV and TB management
    • Assist with phlebotomy duties under direct supervision of a Registered Nurse/Medical Practitioner
    • Collaborates with health facility staff for the provision of HIV/TB prevention strategies, including IPC
    • Assists with elicitation of contacts for newly diagnosed HIV, TB and STI index patients
    • Ensure complete documentation and updating of HIV/TB manual and electronic registers
    • Assists with the provision of technical assistance in HIV counselling and manage and track referral of HIV, TB and STI cases
    • Adheres to all infection prevention and control protocols including medical waste management protocols
    • Supports and expedites decanting of all eligible patients as per SOP including internal PUP management
    • Blood monitoring as per required monitoring intervals
    • Ensure the spread and integration of lessons learnt about how to engage communities to improve the quality of services, retention in care and adherence to treatment
    • Strengthen linkages between the mobile HTS teams and the preferred health facility

    • Work closely with stakeholders and the role players on linking clients from mobile HTS teams into health and ART services to create demand for usage of mobile HTS teams in the community
    • Work closely with DOH to establish community based structures that will create demand for HIV/AIDS care and treatment
    • Coordinate activities with health facilities to ensure that clients referred from the mobile HTS receive the continuum of care within a health facility
    • Meet monthly with health facility managers to provide feedback on the successfully referred clients and the home visits conducted to screen for HIV/TB and STI at the index case home
    • Strengthen relationships and linkages between the mobile HTS teams and the health facilities
    • Community mobilization for HIV/AIDS prevention, care and treatment, PMTCT and TB/HIV services
