Description

To support, at facility level, the clinicians and the patient administration processes with the objective to recruit patients to the CCMDD / Collect & Go Program, according to selection criteria as communicated by the Clinic Manager from time to time.

Requirements

     Minimum Required Qualifications and Experience

• Grade 12
• HIV/AIDS management course
• Marketing and brand activation experience
• Self-starter and innovative individual who can drive targets
• Experience in management of patient records and patient files
• Experience in a public health setting desirable
• Understanding of the CCMDD program

      Key Performance Areas

• Ensure patients understand the CCMDD/Collect & Go program and benefits to them
• Ensure patients understand the process and criteria to use the Collect & Go as a preferred pick-up point
• Train patients how to operate a Collect & Go and other relevant information on queries about Collect & Go
• Provide supporting materials to patients in assisting the recruitment process
• Ensure that supporting materials are in sufficient quantities at the facility and ordering of materials where needed
• Keep a log to account for distribution of marketing material and other promotional items
• Patients that understand the CCMDD/Collect & Go program.
• Increased referral of patients to the CCMDD/Collect & Go program from referral facilities.
• Ensure that patients identified as eligible by clinicians receive their appointment dates for next collection
• Support consenting patients to use CCMDD/Collect & Go program
• Ensure patient understands the pick-up cycle
• Handle additional patient queries
• Escalate problems through appropriate channels
• A seamless and smooth process for enrolment of patients
• Buy-in and positive feedback from site staff and clinicians
• Increased referral of patients to the CCMDD/Collect & Go program
• Assist with loading patient parcels delivered to Collect & Go sites where necessary
• Liaise with site staff and call centre in tracking and following up with enrolled patients
• Inform REP-IT on any technical issues at Collect & Go sites
• Support the process of co-ordinating and preparing scripts that need to be sent for capturing
• Communicating with automated sites, REP-CC and other relevant partners to ensure patients are serviced optimally
• Support the Site Supervisor in collecting data and compiling relevant reports
• Support any research activities conducted  
• High quality data to inform reports.
