    Outreach Officer X4 (Onsite) - Makhado

    Duties and Responsibilities include but not limited to:
     

    • Work with the community-based team to ensure efficient delivery of TB services.
    • Liaise with the Programme linkage officer to trace and start treatment for newly diagnosed TB patients.
    • Ensure proper referral of TB contacts and diagnosed individuals for treatment and additional services, if needed.
    • Support in identifying and testing TB contacts and initiate those eligible for TPT.
    • Conduct home visits for people diagnosed with TB.
    • Provide essential counselling when required.
    • Collaborate with Programme and DOH facility-based staff to trace patients who have become LFTU and link them back to care.
    • Work alongside the Programme Community Engagement Officer in supporting awareness campaigns and promoting health services.
    • Engage daily with hospital staff, health care facilities, and programme teams to ensure efficient linkage of patients to care.
    • Support DOH healthcare facilities and outreach teams in contact tracing efforts.
    • Partner with DOH and other bodies in community outreach campaigns.
    • Collaborate with the TB team in enhancing the quality of care, retention, and adherence to treatment for TB patients.
    • Participate in capacity-building training programmes.


    Requirements:

    • Certification as an Enrolled Nurse from a recognised institution.
    • Counselling certificate an added advantage
    • At least one year of experience in TB or HIV Programme as an Outreach Officer or a role of equivalent responsibility.
    • Demonstrable work experience in both HIV and TB programme implementation.
    • Basic knowledge of Tuberculosis (TB) is an added advantage.
    • Strong interpersonal and communication skills, with an ability to work with diverse populations.
    • Proficiency in maintaining patient confidentiality and understanding of patient rights.
    • Ability to work under pressure and meet tight deadlines.
    • Familiarity with community-based healthcare outreach programs.
    • Bilingual or multilingual capabilities, depending on the community served, can be an advantage.
    • Team player with excellent organisational and multitasking skills.
    • Ability to collaborate effectively with a diverse group of professionals and community members.
    • A valid driver's license.
