    Professional Nurse Coordinator, Metro and Rural Health - Cape Town Metro

    Purpose of the position:

    • To provide a comprehensive Wellness mobile health service by providing HIV testing and counselling services linked to screening and diagnosing clients for TB/STI’s. Provision of treatment and clinical care to HIV & TB patients and ensure successful linkage to care and adherence with specific focused on key populations

    Minimum Requirements    

    • Degree/Diploma in Nursing qualification
    • Registration with the South African Nursing Council as a Professional Nurse
    • At least 2 years' experience in nursing after registration 
    • Clinical HIV management experience is mandatory
    • Previous coordination/management role and experience working with key populations advantageous 
    • Dispencing license would be an advantage
    • Code 8 Drivers licence
    • Clear criminal record
    • Proficiency in Microsoft Office
    • Proficiency in English and isiXhosa; fluency in Afrikaans is advantageous

    Duties and Responsibilities    

    • Coordinate and conduct outreach HIV, TB and STI services in the community, workplace, correctional services, farms schools and DOH facilities  
    • Coordinate peer-led community outreach teams to provide services to key populations and mentor reporting staff on clinical elements of health education and intervention cascades.
    • Conduct health screening, testing and provide therapeutic or preventative care, where relevant in terms of HIV, TB, STIs and other relevant emerging conditions
    • Maintain relations with relevant WCG stakeholders in terms of commodity sourcing, establishing, and maintaining referral networks and reporting activities in accordance with WCG requirements

    Provide an enhanced appropriate package of treatment care and support of clients diagnosed with HIV, TB, STI’s, and other health related conditions 

    • Order monthly stock of commodities and medication and maintain the appropriate stock management and supply chain systems, including relevant drug registers.
    • Conduct physical examination and proper history taking on clients seen in the community (or at a facility as required)
    • Provide treatment where possible or facilitate successful referral to sensitised facilities for onward care.
    • Ensure successful referrals and linkage to care of 95% of key populations affected by HIV or TB

     Provide technical assistance in service delivery and reporting

    • Train implementation teams to follow SOPs, care pathways and complete all relevant source documents and ensure reporting to relevant stakeholders and health information systems
    • Ensure prompt tracing of lost to follow up clients through telephone contact, home visits, inclusive of the provision of enhanced adherence counselling or referral for advanced clinical care, as required
    • Conduct sensitisation training and secure the provision of KP friendly services at nominated sites, in consultation with WCG and other officials.
    • Ensure decentralised care and distribution of medication within the community setting, where possible.
    • Ensure period reporting is conducted in line with WCG and internal organisational norms and standards

    Collaborate with stakeholders to scale up TB/HIV service provision to key populations

    • Ensure effective collaboration with various implementing partners or service providers within the KP network and feasible referral pathways are established, catering for the unique needs of key populations
    • Ensure effective community entry with appropriate stakeholder inclusion and involvement including law enforcement agencies, community leaders and government officials
    • Meet periodically (monthly with facility stakeholders and quarterly with other stakeholders) re current and future expanded service provision

    Maintain relations with health services and other NGO’s working to support program

    • Attend HAST Information meetings
    • Adhere to waste disposal procedures
    • Adhere to the needle stick injury policy
    • Report quarterly on program progress
    • Meet monthly with facility managers on sites activities
