    Community Health Worker, Metro and Rural Health - Cape Town

    Purpose of the position:

    • To conduct out-reach activities in the sex worker community by providing current accurate health information, mobilisation of SWs to the mobile unit and linkage to care.

    Minimum Requirements    

    • Grade 10
    • Accredited or recognised counselling certificate or feasible opportunity to obtain this during the probation period
    • Experience in a key populations field advantageous
    • Previous experience as Community Health Worker or HIV tester advantageous

    Duties and Responsibilities    
    Mobilisation

    • Identify relevant key population hotspots for mobilisation (PWUD, FSW, MSM, TG, farm workers)
    • Mobilise key population workers to access appropriate mobile and facility health and social services

    Community interventions

    • Conduct outreach into community hotspots to provide male and female condom demonstration, health education (risk and harm reduction, prevention and treatment options)
    • Conduct health screening including HIV (inclusive of assisted self screening), TB and STIs
    • Distribute IEC material, male and female condoms, lubricants and other commodities, as required
    • Conduct point of care testing e.g. finger prick rapid HIV testing, as required
    • Appropriately link client to nurse-led testing/care as required

    Follow-up and referral  

    • Assist team in follow-up of referral of all new cases – HIV, TB, STIs and other health issues
    • Successfully refer key populations workers to mobile and facilities services
    • Assist clients with appropriate referrals for other issues related to psychosocial and/or escalated care/intervention (e.g. paralegal, post-violence or substance use supportive care etc.)
    • Set up and facilitate peer support groups or community advisory meetings as appropriate

    Health education

    • Provide accurate and current health information to sex workers using a variety of accepted strategies
    • Assist with the development of new health promotion materials

    Build and maintain relations

    • Maintain relations with health services managers, indoor managers/owners and other gate keepers/stakeholders (including the client network)

    Record management

    • Use the attendance register for counting number of male and female present during support group or community advisory group meetings
    • Utilise appropriate source documents, aligned to WCG requirements, to document outreach efforts and interventions
    • Utilise appropriate internal tools for data or performance reporting
    • Secure clinical information appropriately and submit timeously

    Monitoring & Evaluation and Reporting

    • Monitor and evaluate the impact of outreach activities and compile a summary of activities to be submitted to the line manager on a weekly basis
    • Report back to the Line Manager any positive or negative feedback from clients and the general community

    Professional Nurse Coordinator, Metro and Rural Health - Cape Town Metro

    Purpose of the position:

    • To provide a comprehensive Wellness mobile health service by providing HIV testing and counselling services linked to screening and diagnosing clients for TB/STI’s. Provision of treatment and clinical care to HIV & TB patients and ensure successful linkage to care and adherence with specific focused on key populations

    Minimum Requirements    

    • Degree/Diploma in Nursing qualification
    • Registration with the South African Nursing Council as a Professional Nurse
    • At least 2 years' experience in nursing after registration 
    • Clinical HIV management experience is mandatory
    • Previous coordination/management role and experience working with key populations advantageous

    Duties and Responsibilities    

    • Coordinate and conduct outreach HIV, TB and STI services in the community, workplace, correctional services, farms schools and DOH facilities  
    • Coordinate peer-led community outreach teams to provide services to key populations and mentor reporting staff on clinical elements of health education and intervention cascades.
    • Conduct health screening, testing and provide therapeutic or preventative care, where relevant in terms of HIV, TB, STIs and other relevant emerging conditions
    • Maintain relations with relevant WCG stakeholders in terms of commodity sourcing, establishing, and maintaining referral networks and reporting activities in accordance with WCG requirements

    Provide an enhanced appropriate package of treatment care and support of clients diagnosed with HIV, TB, STI’s, and other health related conditions 

    • Order monthly stock of commodities and medication and maintain the appropriate stock management and supply chain systems, including relevant drug registers.
    • Conduct physical examination and proper history taking on clients seen in the community (or at a facility as required)
    • Provide treatment where possible or facilitate successful referral to sensitised facilities for onward care.
    • Ensure successful referrals and linkage to care of 95% of key populations affected by HIV or TB

     Provide technical assistance in service delivery and reporting

    • Train implementation teams to follow SOPs, care pathways and complete all relevant source documents and ensure reporting to relevant stakeholders and health information systems
    • Ensure prompt tracing of lost to follow up clients through telephone contact, home visits, inclusive of the provision of enhanced adherence counselling or referral for advanced clinical care, as required
    • Conduct sensitisation training and secure the provision of KP friendly services at nominated sites, in consultation with WCG and other officials.
    • Ensure decentralised care and distribution of medication within the community setting, where possible.

    Collaborate with stakeholders to scale up TB/HIV service provision to key populations

    • Ensure effective collaboration with various implementing partners or service providers within the KP network and feasible referral pathways are established, catering for the unique needs of key populations
    • Ensure effective community entry with appropriate stakeholder inclusion and involvement including law enforcement agencies, community leaders and government officials

    Track and Trace Driver - Mthatha

    IT Administrator - eThekwini District

    Purpose of the position:

    • To apply professional experience and training in information technology and network communications to provide basic technical support for a large number of users. To support the users with email support, mobile device support and basic network/desktop support.

    Minimum Requirements    

    • 2-3 years’ experience is desirable
    • Basic desktop support experience essential
    • Working knowledge of routers, access points and desktops
    • N+ Certification preferred
    • Clear criminal record

    Duties and Responsibilities    
    Desktop support

    • Install, update, and configure computers and tablets
    • Software installations
    • Troubleshoot, diagnose, and resolve problems relating to hardware and software
    • Setup of mobile devices and new laptops/desktop computers on an Azure domain and routine maintenance of computer hardware and software Supporting of Windows & Apple Mac computers
    • End-user support to staff within the region
    • Telephonic support to staff at branches in the region
    • Assists in providing network and remote connectivity hardware/software support using Teamviewer
    • Setting up and configuration of technical equipment such as Email, Projectors, Microsoft Teams, etc.

    Network support

    • Establishes and maintains user accounts, and profiles
    • Troubleshoot site connectivity and VOIP system
    • Reporting faults to the service provider, following through, and resolution
    • Monitor and support router on various carries: Fibre, Microwave, LTE
    • Configure and support wireless network
    • Setup email on mobile devices
    • Printer installation, setup, and support
    • Basic Microsoft Office 365 knowledge
