Social Worker- Somerset West at Faircape Group

Responsibilities

• Conduct psychosocial assessments and contribute to discharge planning.
• Provide counselling, emotional support and mediation for patients and their families.
• Coordinate multidisciplinary care and facilitate referrals to appropriate community and healthcare services.
• Identify and address barriers to recovery, treatment adherence and successful discharge.
• Maintain accurate clinical documentation and participate in multidisciplinary team discussions.
• Advocate for patients' social, emotional and practical needs throughout their care journey.

Requirements

• Current SACSSP registration.
• Professional indemnity insurance.
• Minimum of 3 years' social work experience.
• Experience in hospital social work and case management.
• Training and experience in palliative care (essential).
• Strong report writing and time management skills.
• Ability to work collaboratively within a multidisciplinary team.
• Willingness to travel for outpatient consultations.
• Experience in geriatrics, oncology and curatorship will be advantageous.

Compensation

• 60% commission on services rendered.
