Document patient assessments, care plans, interventions, and outcomes accurately and timely.
Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives as required.
Minimum Requirements:
Registered Nurse qualification with current registration with the South African Nursing Council (SANC).
Minimum of 3 years of experience in case management within a private hospital setting.
Strong understanding of clinical processes, patient care pathways, and healthcare delivery systems.
Excellent assessment, planning, and organizational skills.
Effective communication and interpersonal skills, with the ability to collaborate with diverse stakeholders.
Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
Proficiency in electronic medical record systems and Microsoft Office applications.
