Case Manager- Randburg / Contract at BlueMarble

Key Responsibilities:

• Assess patient needs, preferences, and goals to develop comprehensive care plans in collaboration with healthcare providers.
• Coordinate and facilitate the delivery of patient care services, including medical treatment, therapies, and support services.
• Monitor and evaluate patient progress, outcomes, and satisfaction with care plans, adjusting as necessary to ensure optimal outcomes.
• Communicate and collaborate effectively with healthcare providers, patients, families, and external stakeholders to facilitate care transitions and continuity.
• Advocate for patients' rights, preferences, and needs throughout the healthcare journey.
• Document patient assessments, care plans, interventions, and outcomes accurately and timely.
• Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives as required.

Minimum Requirements:

• Registered Nurse qualification with current registration with the South African Nursing Council (SANC).
• Minimum of 3 years of experience in case management within a private hospital setting.
• Strong understanding of clinical processes, patient care pathways, and healthcare delivery systems.
• Excellent assessment, planning, and organizational skills.
• Effective communication and interpersonal skills, with the ability to collaborate with diverse stakeholders.
• Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
• Proficiency in electronic medical record systems and Microsoft Office applications.
