    Clinical Nurses (Community Hospital Interface Program)

    Metro South Health@Home are looking for experienced Clinical Nurses to join their Community Hospital Interface Program (CHIP) team. You will provide expert clinical leadership in patient’s/resident’s/client’s assessment, care planning and practice and role model to and collaborate with the healthcare team. The CHIP service assists with hospital avoidance by reducing the length of stay decreasing the risk of representation, supporting Commonwealth Emergency Department Length of Stay (ELOS) targets and providing holistic assessment and referral processes within a multidisciplinary framework resulting in optimal patient outcomes and cost efficiency for the health service.

    About The Role

    The Clinical Nurse (CN) role provides advanced clinical and problem-solving skills, expert planning and coordination skills in the clinical management of CHIP patients with complex care needs, ensuring quality standards are met. This role works autonomously within the Healthcare Team structure and is pivotal in the planning, managing and delivering of CHIP specialist nursing care. The CN provides clinical leadership for professional and clinical practice, education and research. The role supports in management activities including portfolio responsibilities.
