Case Manager Case Management at Lenmed Private Hospitals

We are looking for a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle and is expected to assess, plan, implement, monitor and evaluate actions required to meet the client’s health and human services needed.

Key Performance Areas: (include but are not limited to)

• Accurate and timeous communication and follow up with medical aids.
• Effective communication with other role players in the healthcare team.
• Provide appropriate clinical information to Medical Aids in order to update LOS/LOC
• Effective and timeous communication with the member of the fund when appropriate.
• Application of clinical knowledge for effective ICD 10 and CPT coding of patient records.
• Utilize all available resources for accurate and appropriate coding
• Daily visits to all medical aid patients
• Control costs – Effectively Manage Fixed Fee Cases.
• Consult with all service suppliers regarding treatment for patients.

Post Requirements: (include but are not limited to)

• Registration with the South African Nursing Council as a Registered Nurse/Enrolled Nurse
• Minimum 3-year experience as a Case Manager in a Multi-Disciplinary Acute Hospital.
• SAP Experience
• ICD10 / CPT4 Coding Qualification
