    Enrolled Nurse (Nkangala)

    Description

    • Pull out files for TX_NEW -Flag files for patients not initiated on IPT
    • Call all eligible patients and take them to the DSD Nurse for initiation.
    • Make sure that all initiated patients have been captured on Tier.net.
    • Increase IPT Initiation among patients currently on ART by using the viral load due lists and patients on adherence clubs.
    • Request files for patients with no outcomes, audit them and ask the Data Capture to update outcomes on TIER.NET
    • Every month request a list of patients that needs repeat script for IPT and ensure that the treatment is continued for the prescribed duration according to guidelines.
    • Counselling of patients in their own language, get consent and offer rapid HIV testing.
    • Refer all positive patients to the Professional Nurse for initiation on the same day (Handshake approach)
    • Link mother baby pairs to care through identification of HIV positive mothers and PCR positive babies and refer them for initiation of ART
    • Go through the TB room and make sure that all TB presumptive /diagnosed patients have been tested for HIV – if not make note to test them on their next visits.
    • Screen patients for STI, TB by asking the 4 screening questions and refer patients accordingly
    • Support all Self Screening activities happening within and in vicinity of your facility.
    • Ensure that the Risk Profiling tool is used in your facility.
    • Support the Professional Nurse with drawing of Viral load due bloods
    • Create index testing contact list, call the partners as according to your list using the facility phone or any alternative provided.
    • Collaborate with Right to Care Community Partners, WBHCOTS and HBC teams to help trace all Index contacts on your list

    Requirements

    Minimum requirements

    • Enrolled Nursing Diploma/Certificate
    • 2-3 Years as an Enrolled Nurse in the NGO/Community or a Hospital Environment 
    • Knowledge of HIV/AIDS

    Data Capturer (Nkangala )

    Description

    Capturing of HIV patient data

    • Capture all relevant patient data on site on the LYNX APP
    • Reach daily targets for number of files captured and/or updated, as agreed upon with Line Managers.
    • Record stats relating to daily work performance ('data entry stats'), to enable line manager to manage the employee's work performance.
    • Always respect patient confidentiality.

    Cleaning of HIV patient data

    • Compile daily Blitz Reports.
    • Assist with data cleaning whenever required.

    Ongoing skills development

    • Take responsibility for own professional growth and development.
    • Obtain guidance from supervisors / managers in case of uncertainty about patient data entry or other issues.
    • Attend compulsory in-house trainings to maintain and increase professional knowledge and skills.
    • Identify and attend further trainings, as relevant to the job, to maintain and increase professional knowledge and skills.
    • Collaborate with data team in compiling reports (e.g. PEPFAR reports and DoH reports, on VCT, TB,PMTCT, Care, ART, etc.)

    System Management

    • Assist M&E officer & Senior data capturer with most essential duties during periods of leave, where this does not interfere with regular
    • Assist clerks during unexpected staff shortages for brief periods of time.
    • Assist with DM duties during unexpected long absence of DM during reporting deadlines   

    Requirements

    Required Minimum Qualifications and Experience

    Professional Nurse (Nkangala )

    Description

    Patient Care

    • Assess, examine and diagnose patient’s clinical conditions and triage.
    • Do relevant blood test and observations / vital signs.
    • Interpret HIV test results, follow up on inaccuracies
    • Assist with the process and implementation of VCT(Voluntary Testing and Counselling) / HCT (HIV Counselling and Testing).
    • Design and implement nursing care plans, monitor and evaluate patient’s progress.
    • Perform pap smears on all HIV infected patients yearly monitor and refer appropriately.
    • Assist with the process of implementing PMTCT (Prevention from Mother to Child Treatment) and TB (Tuberculosis) programmes / services at the site
    • Implement NIMART as mandated in Section 38A
    • Record keeping of all relevant patient information on files / (Therapy Edge).
    • Refer appropriately.

    Staff Development

    • Supervise and coordinate activities of all sub-ordinates.
    • Participate in the site in-service training program

    Administration Duties

    • Compile monthly and daily reports of ART patients.
    • Ensure efficient coordination of the anti retroviral site, its referrals networks and ensure sustenance of the ART Programme

    Research

    • Keep abreast with present developments in the field
    • To participate in research projects relevant to the specific field
    • Generate relevant reports and do follow ups on inaccuracies

    Requirements

    • Diploma in General, Community and Midwifery Nursing
    • 2 years as a Registered Nurse in NGO/Community Clinic Or a Hospital Environment

    Desirable Qualifications and Experience

    • Diploma\ Degree in General, Community and Midwifery   Nursing

    • 3 years’ experience as a Registered Nurse Community Nursing Health setting or Hospital Setting

    Program Improvement Officer (Gert Sibande)

    Description

    Technical Support & Clinical Quality Improvement

    • Provide on-site technical assistance to facility staff on CCMT, HIV, TB, IMCI and PHC programs.
    • Conduct clinical file audits, root-cause analyses, and quality-of-care assessments at facilities.
    • co-create and monitor quality improvement plans with facilities management.
    • Support implementation of evidence-based interventions aimed at improving clinical outcomes of the different DoH Programs
    • Mentor clinicians and nurses on updated NDoH guidelines, SOPs, algorithms, and protocols.
    • Facilitate case discussions, mortality reviews, and program performance reviews.

    Monitoring, Evaluation & Data Use for Action

    • Conduct routine data verification, indicator tracking and performance monitoring using DHIS, TIER.Net, SVS, SyNCH, HPRS, ETR, EDR Web, and facility registers.
    • Analyse program data to identify gaps and trends and develop corrective action plans.
    • Promote data use at facility level through dashboards, programme review meetings and on-site coaching.
    • Support facilities to prepare for Nerve Centre, District Reviews, or performance reporting.

    Health Systems Strengthening & Program Optimization

    • Strengthen patient flow systems
    • Facilitate integration of HIV/TB/IMCI/STI/Family Planning services within PHC platforms.
    • Lead the implementation of quality improvement projects on Facility Improvement Plans (FIPs)
    • Lead the implementation of quality improvement projects on Ideal clinic realisation and maintenance (ICRM/ICSM)
    • Coordinate formative assessments and facilitate gap analysis guided by key programme goals and objectives 
    • Project selection: Identify project options best suited to address identified gaps, needs and problems
    • Prepare project plans including FIP, resource requirements and task assignment to staff
    • Periodic project monitoring and coordination.
    • Participation in all review meetings

    Capacity Building & Mentorship

    • Deliver on-site and classroom-based training aligned to NDoH clinical guidelines and programmatic priorities.
    • Build capacity of facility teams to implement standardised clinical, data, and administrative processes.
    • Provide supportive supervision to operational managers and frontline staff.

    Stakeholder Management

    • Building healthy relationship with the facility managers, sub-district DoH Program coordinators, Supervisors, PHC Managers, Partner organisations and district health management team.
    • Participate in District and Sub-District program meetings, technical working groups, and review forums.
    • Support coordination of community-facility collaborative activities including supporting the work of WBPHCOTs, CBOs and other partners
    • Ensure to maintain communication flow with the district management on important programmatic issues and in tracking implementation progress

    Requirements

    Required Minimum Qualifications and Experience

    • Diploma or Degree in Nursing or a health-related qualification at a tertiary level.
    • 3 year’s experience in an NGO or District Department of Health setting

    Desirable Qualifications and Experience

    • Post-graduate qualification in Public Health or Management
    • 5 year’s experience in implementing Health Programmes
    • At least 3 years’ experience in clinical program management including HAST programs, Monitoring & Evaluation, report writing and presentations at Sub-District and District levels.
