    Social Auxiliary Worker (Social Development)

    Purpose of the position:

    • To provide professional and holistic social work services by assessing the needs of patients, families, carers and children with TB and/or HIV. The role involves implementing strategies and processes to meet these needs.

    Minimum Requirements    

    • Grade 12 or equivalent
    • Certificate in Social Auxiliary Work from a recognized college or institution
    • Minimum 2 years relevant experience
    • Registration with the South African Council for Social Service Professions (SACSSP)
    • Proficiency in English and isiXhosa languages
    • Clear criminal record
    • Computer literate in MS office advantageous

    Duties and Responsibilities    

    • Assists in the coordination of comprehensive psychological assessments of patients and their families at Anti Retroviral Clinics/ Mobile
    • Educates patients and their families
    • Informs patients and their families on eligibility criteria for social support grants
    • Networks with all relevant stakeholders and implements an effective referral system to ensure effective service delivery
    • Lobbies and advocates for patient’s needs
    • Provide appropriate interventions and/or feedback at the case management meeting on the needs arising
    • Follows up on referrals from hospital staff and/or teams
    • Submits monthly reports to the supervisor

    Project Coordinator

    Job Description:

    • The Project Coordinator is responsible for coordinating and supporting the overall functioning of a People Who Use Drugs (PWUD) Programme at a district level.

    Minimum Requirements    

    • Degree in social sciences/health sciences
    • Certificate in project coordination/management or relevant experience
    • 5 years working in HIV prevention and/or treatment field
    • 5 years working in community engagement field
    • Experience in a programme that provides services to PWUD is essential
    • Experience working in a programme that provides services to other vulnerable or key populations is beneficial (e.g. street people, sex workers)

    Community Coordinator

    Purpose of the position:

    • The Community Coordinator will provide operational management of the HIV/TB/STI comprehensive prevention package of service. The package focuses on HIV and TB prevention, in active case finding & same day ART initiations and TB preventive therapy (TPT). The HIV newly diagnosed and the TB contacts are to be case managed by the mobile team. The mobile teams provide screening for HIV, TB, Covid-19, STI’s, non-communicable diseases, condom promotion and distribution with navigation of referrals into a facility and laying the HIV negative girl & women with priorities population prevention programmes. They are responsible for executing the Index Testing modality & sexual partner/s case finding with the biological children =<19 years old and the HIV self-screening modality with distribution in primary assisted and secondary unassisted methodology.

    Minimum Requirements:    

    • Diploma in Nursing or Bachelor's degree (B. Cur) in Nursing
    • HIV Management Diploma or equivalent combination of education and experience
    • NIMART and preferred dispensing licence
    • Current SANC registration
    • 3 yrs experience in HAST programme coordination
    • Valid code 08 driver’s licence
    • Clear criminal record

    Duties and Responsibilities:    

    • Provides operational and technical support to the Nurse Clinician in the CB HTS programme
    • Meet weekly and monthly with Programme Director: HTS and Care & Treatment
    • Index testing and HIVSS modalities with same day ART initiations of HIV newly diagnosed client, sexual partners & biological children, commence adherence support and monitor ART care.
    • Meet quarterly with DOH (sub district, district and province) managers, community stakeholders and other service providers, so as to engage them in scaling up the THC package of service provision

    Social Auxiliary Worker - Amathole

    Purpose of the position: 

    • To provide psychosocial services under the supervision of a social worker within the context of the Care and Treatment programme focusing on people living with HIV (PLHIV), adolescent girls and young women (AGYW), adolescent boys and young men (ABYM) who are at risk of interrupting treatment. This will include screening and risk-based assessment using the available tools, and referral to a social worker for therapeutic social work services.
    • The social auxiliary worker (SAW) will also conduct adherence support within the framework of adherence guidelines addressing disclosure and adherence issues, mental health, as well as engage with relevant stakeholders such as government departments, and partners in the social and health work fields.

    Minimum Requirements:    

    • Grade 12 or equivalent

    Case Officer

    Purpose of the position:

    • Responsible for implementing and monitoring the Case Management Model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care.

    Minimum Requirements    

    • Grade 12 certificate
    • Enrolled nurse qualification will be an added advantage (most suitable for pediatric streams)
    • At least 2 years’ relevant experience in counselling and testing

    Duties and Responsibilities:   

    • Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments
    • Capture clients with high viral loads, pediatric clients, pre-ART clients and pregnant women
    • Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care
    • Issue routine reminders according to the schedule of appointments by sending SMS reminders
    • Responding to messages from clients and crafting an appropriate and professional response
    • Refer patients for enrolment in support groups according to the patient’s preference

    Peer Educator

    Purpose of the position:

    • To conduct out-reach activities in the sex worker community by providing current accurate health information, mobilisation of SWs to the mobile unit and linkage to care

    Minimum Requirements    

    • Grade 10 
    • Accredited or recognised counselling certificate

    Proffessional Nurse Counsellor

    Purpose of Position

    • The Professional Nurse (PN) is responsible for comprehensive, integrated cascade of care, inclusive of diagnosing, treating, monitoring and evaluating communicable and infectious diseases. The PN will consult and provide preventative, curative and rehabilitative treatment/health care to sex workers on various clinical aspects (Adult Curative care inclusive of communicable, non-communicable diseases).

    Duties and Responsibilities    

    • Conduct regular meetings to discuss progress and address challenges to continuously improve programme implementation.
    • Liaise with public health facilities and other relevant role-players – attending meetings etc.
    • Work with the Site Manager and the outreach staff to ensure the sensitisation of service providers to the needs of sex workers
    • Provide primary health care (PHC) services and comprehensive HAST services
    • Work with the Site Manager to ensure the implementation of the programme's operational plan
    • Weekly quality assurance cycle for reliable and accurate HIV patient results
    • Consolidate data and other information from the team, prepare and submit monthly and quarterly reports to M&E Officer and Regional Coordinator
