Assist patients in their daily activities, including personal hygiene, eating, dressing, and mobility, while maintaining their dignity and privacy.
Implement prescribed rehabilitation programs under the guidance of healthcare professionals, including physiotherapists, occupational therapists, and speech-language pathologists.
Monitor and document patients' progress and behavior, reporting any concerns or changes to the interdisciplinary team.
Administer medication, following established protocols and guidelines, as applicable.
Create a safe and supportive environment for patients, ensuring the facility's policies and procedures are adhered to at all times.
Assist in the implementation and execution of therapeutic activities and exercises designed to improve mobility, strength, coordination, and functionality.
Provide emotional support and companionship to patients, fostering a therapeutic relationship based on trust and empathy.
Collaborate with the interdisciplinary team to develop and modify individualized care plans based on patients' needs and goals.
Guide and educate patients and their families regarding their rehabilitation process, including techniques for home care and ongoing support if required.
Maintain accurate and up-to-date records of patient care, progress, and interventions.
Required Skills and Qualifications:
High school diploma or equivalent is required. Certification in a healthcare-related field (e.g., Certified Nursing Assistant, Personal Support Worker) is preferred.
Proven experience as a care worker or similar role is desirable.
Basic understanding of rehabilitation principles, techniques, and equipment.
Strong interpersonal skills with the ability to communicate effectively and compassionately with patients, their families, and the interdisciplinary team.
Good observational and documentation skills to accurately monitor and report patient progress.
Ability to prioritize, multitask, and adapt to changing circumstances in a dynamic environment.
Physical stamina and dexterity to assist patients with mobility, transfers, and activities of daily living.
Knowledge of infection control practices and a commitment to maintaining a clean and safe care environment.
Ability to work independently, as well as part of a team, with excellent collaboration and problem-solving skills.
Flexibility to work various shifts, including evenings, weekends, and holidays.
