    Tracing Officer - Polokwane

    Job Description

    We are looking for a Tracing Officer to do comparison of the Tier. Net missed appointment list and the source document (patient’s files) for verification. To improve adherence to care of TB/HIV patients to PHC facilities by tracing the missed appointment clients back to care telephonically through brief adherence counselling. To facilitate referral of all missed appointments of the TB/HIV programme who can’t be traced telephonically through the WBPHCOT. To give a detailed report of tracing done to facility managers per traced facility and do improvement plan.

    Key duties and responsibilities:

    • Facilitate all tracing processes (telephonic and physical) of the priority facilities.
    • Liaise with facility staff and WBPHCOT team leaders for the tracing process and report for file and Tier.Net update.
    • Assist the PHC facility with telephonic tracing of missed appointments and defaulters.
    • Assist the WBPHCOT’s to trace defaulters within the communities.
    • Compilation of all clients that were not reached telephonically to the WBPHCOT register for physical tracing.
    • Completion of the WBPHCOT register at the facility when giving the tracing report and feedback to the facility staff for outcome on Tier.

    Qualifications, experience and skills required:

    • Grade 12 (Matric)
    • 3 years of experience working with PLWHA especially within the community.
    • Trained in basic HIV counselling and testing.
    • Trained in adherence counselling.
    • Shown affiliation and passion with community-based activities.
    • Strong interpersonal, written, and oral communication skills are required.
    • Ability to speak Northern Sotho (Sepedi).

    Social Auxiliary Worker

    Minimum Qualifications and Experience:

    • Matric/Grade 12 certificate and Social Auxiliary Work certificate recognized by the Health and Welfare Sector Education and Training Authority (HWSETA) and enrolled in Social Work degree.
    • Must have at least 2-5 years’ experience working with PLHIV (people living with HIV).
    • Training in KidzAlive, Integrated Access to Care and Treatment (I-ACT) and NAS (National Adherence Strategy) an advantage

    Skills, Competencies, and Attributes:

    • Must have experience working in HIV/AIDS programmes and supervision of staff.
    • Good presentation and excellent communication skills and good report writing skills.
    • Strong interpersonal and teamwork skills.
    • Proficiency with MS Office, MS PowerPoint and MS Excel.

    Deputy Director: Multisectoral HAST

    Job Description 

    • Social Sciences/ Health Information Sciences or any equivalent degree
    •  At least 5-10 years’ experience working in HIV/TB and STI programmes
    •  At least three (3) years’ experience working in technical leadership, oversights, design and implementation of HIV/TB & STI programmes
    •  Demonstrated technical knowledge and leadership in programme management, strategic planning and policy experience
    •  Excellent interpersonal and client liaison management skills
    •  Good facilitation and communication (both verbal and written) skills
    •  Excellent Analytical and reporting skills
    •  Proactive and problem resolution Skills
    •  General and financial management skills
