    Post Basic Pharmacy Assistant, OR Tambo (Temporary)

    Purpose of the position:

     

    • The PBPA is responsible for strengthening pharmaceutical healthcare services and providing direct service delivery at district facility level. The primary responsibility is to support the Differentiated Care Pharmaceutical Services component of the Central Chronic Medicine Dispensing and Distribution Programme (CCMDD), and the secondary responsibility is to support stock management at facility level. The incumbent will report primarily to the CCMDD Coordinator, and secondarily to the CCMDD Manager.

    Minimum Requirements    

    •  Grade 12

    Duties and Responsibilities    

    • Support the CCMDD programme by ensuring that appropriate CCMDD tools are deployed in focus facilities in support of the program.
    • Verify CCMDD Service Provider data against statistics at the facility level.
    • Monitor the implementation of Service Level Agreements (SLAs) and evaluation of performance of contracted CCMDD Service Provider and Pick-up Points.
    • Support the implementation of corrective pharmaceutical actions in cooperation with the CCMDD Coordinator, facility staff and the project team.
    • Relationship management and on-going liaison with contracted CCMDD Service Provider and district stakeholders, as well as close collaboration with other project teams within the district.

    NIMART Nurse, OR Tambo (Temporary) OR Tambo

    Purpose of the position:

    • The incumbent will provide comprehensive HIV and TB clinical services, integrated cascade of care, inclusive of diagnosing, treating, monitoring and evaluating communicable and infectious diseases. They will be responsible for the delivery of HIV and TB clinical services to patients at primary health care clinics in compliance with the project’s HIV and TB service profile and reporting requirements. They will work collaboratively with outreach teams and contracted community based organisations (CBOs) in monitoring adherence of decanted patients, e.g. reporting non-collection of medicine parcels at Central Chronic Medicine Dispensing and Distribution (CCMDD) pick-up points.

    Minimum Requirements

    • NIMART trained and certified
    • Adult Primary Care (APC) is essential

    • The incumbent will provide comprehensive HIV and TB clinical services, integrated cascade of care, inclusive of diagnosing, treating, monitoring and evaluating communicable and infectious diseases. They will be responsible for the delivery of HIV and TB clinical services to patients at primary health care clinics in compliance with the project’s HIV and TB service profile and reporting requirements. They will work collaboratively with outreach teams and contracted community based organisations (CBOs) in monitoring adherence of decanted patients, e.g. reporting non-collection of medicine parcels at Central Chronic Medicine Dispensing and Distribution (CCMDD) pick-up points.

    Minimum Requirements    

    • Diploma in Nursing or Bachelor's degree (B. Cur) in Nursing
    • Valid SANC registration
    • NIMART trained and certified
    • Adult Primary Care (APC) is essential 
    • 3 years’ experience in Primary Health Care programme
    • 2 years’ experience in nursing post registration
    • 2 years’ experience in HIV and TB programmes
    • Experience with NIMART, CCMDD, Sexual Reproductive Health (SRH), Maternal and Child Health (MCH)
    • Dispensing licence will be an added advantage
    • Willingness to travel as and when required
    • Proficiency in Microsoft Office package
    • Ability to communicate in IsiXhosa and English or another South African language
    • Valid code 08 driver’s licence
    • Clear criminal record

    Duties and Responsibilities    

    • Provide services using evidence-based HIV and TB interventions in all streams according to the DoH policy, guidelines and SOPs.
    • Optimise HTS at facilities
    • Implement the Universal Test and Treat (UTT) policy according to the latest guidelines to support attainment of UNAIDS 95-95-95 targets
    • Monitoring referrals and ensuring successful referrals are validated 
    • Support the Ideal Clinic Realisation and Maintenance (ICRM) programme and quality improvement interventions at the facility
    • Implement an effective appointment system and differentiated care models of the decanting programme
    • Promote patient decanting of stable patients by creating demand for the CCMDD service and TLD transition and identifying eligible patients
    • Participate in facility QIP using bottle-neck analysis
    • Keep abreast with new clinical developments e.g. training, in-service training, reading journals, policies, and procedures related to job
    • Implement case management and support patient retention in HIV and TB programme
    • Participate in COVID-19 screening, referral and ensure compliance with infection control measures
    • Record and trace patients interrupting treatment working with coaches and case officers and verify lists of ‘not confirmed as linked to care’
    • Conduct weekly reviews to clear ART waiting patient lists and trace clients (adult and paediatric) for treatment initiation
    • Review cohort reports and facilitate tracing of PCR-positive babies and HIV positive women for treatment initiation
    • Monitor viral load uptake and suppression using eLabs
    • Develop an effective system to monitor viral load tests done on a weekly and monthly basis and results for action (rFA)
    • Scale up CCMDD patient registration and prescription on SyNCH
    • Participate in data analytics

    Facilitator - Durban

    Purpose of the position:

    • The purpose of position job is to provide multi-session evidence based structural programmes and become an integral part of the broader combination prevention care and treatment programme in a non-clinical site  and/or community that links the community into care. (This is a contract position renewable based on  funding)

    Minimum Requirements    

    •  Grade 12 / matric 
    •  Accredited / recognised counselling certificate
    •  1-year experience working in the community or completed the THC skills building programme/internship
    •  Facilitation training and beneficiary of PPPrev programme would be an advantage
    •  Computer literate in MS office 
    •  Driver’s licence would be advantageous
    •  Proficiency in English and isiZulu language
    •  Clear criminal record

    Duties and Responsibilities    

    • Introduce evidenced based structural programmes to the facilities and CBOs in the cluster THC is supporting
    • Map out the communities, schools and institutions of higher learning with burden of disease and large numbers of adolescent girls and young women
    • Introduce structural programmes and obtain entry in schools, institutions of higher learning
    • Identify safe spaces in the community where structural programmes can be conducted
    • Conduct vulnerability screening and identify the vulnerable girls to participate in structural sessions
    • Develop a work plan for the upcoming month
    • Provide multi-session structural programmes
    • Records and maintains accurate and detailed information and keeps statistics in order to submit as required.
    • Networks with all relevant stakeholders and implements an effective referral system to ensure effective service delivery;
    • Ensure layering between structural programmes and behavioral and biomedical services
    • Lobbies and advocates for youth in relation to HIV, gender and related issues
    • Attends training and workshops as part of staff development
    • Participates in interdisciplinary team meetings
    • Submits monthly reports to the supervisor
